You can switch Relics during cast, and still get benefit from them without any downsides (like invoking extra GCD). Besure to replace the relics name, with the relic you are replacing.
/cast Healing Wave(Rank 8) /equip Vengeful Gladiator's Totem of the Third Wind
Replace the Vengeful Gladiator’s Totem of the Third Wind with whatever totem you wish to equip on your shaman.
*As of patch 2.4.3 this macro no longer works. Thanks to the commenter that was mistakengly posted on another macro ;)

After the latest patch, I realised that there is a problem with relic switch macro during combat. It seems like you start to heal withs switchşng relik but suddenly got intruption. You only can continue after your second try. And it occurs every time you swicht one heal to another. But out of combat, everything works fine with macro. Couldnt solve why this is happening.
Any advice?

As of 2.4.3 this macro will no longer work, and the switching of relics now cancel the spell cast.
